1) Has anyone reported to you or to your staff any problem or concern regarding students or employees of your institution with regard to alcohol or drugs?

2) Has anyone reported to you or to your staff any problem or concern regarding students or employees of your institution with regards to sex? (This includes any physical contact unwanted or wanted between students and students or students and staff, between members of the same sex or the opposite sex.)

3) Has anyone reported to you or to your staff any problem or concern regarding students or employees of your institution with regard to weapons? (This includes knives, guns, rifles and any other form of weapon.)

4) Has anyone reported to you or to your staff any problem or concern regarding students or employees of your institution with regard to pornographic literature or movies? (This includes possession of any form of adult material like magazines, books, pictures, movies rated R, X, XXX, etc.)

5) Has anyone reported to you or to your staff any problem or concern regarding students or employees of your institution with regard to kashrus?
(This includes the purchase of nonkosher products, the purchase of products from a store which does not have a moshgiach on staff, the possession of nonkosher products or eating at nonkosher places.)

6) Has anyone reported to you or to your staff any problem or concern regarding students or employees of your institution with regard to stealing?

7) How many brothers / sisters from the same family do you have enrolled in your school? How many students left your school last year before the school year ended? (Please provide me with a list of last year's beginning & ending enrollment.)

8) What percentage of this institution's total enrollment is from the local community?
{Please provide me with the names and phone numbers of the local congregations.)

9) Does your administration require background checks on all employees including rabbium? Who does the background check? .

10) How do you select your Board of Directors?
(Please send me a list of the Board of Directors.)

11) Does your institution require background checks on all its students?

12) Have other parents sent an inquiry list like I have?

13) Are there any reported incidents regarding your institution and local authorities?

14) Has the institution ever lost its accreditation? Are you presently in good standing? What are your strengths as you see them? What are your weaknesses as you see them? Are all your instructors state certified?

15) May I please have the names and phone numbers of the last five employees no longer employed with your institution and their reasons for leaving?

16) What are your institution's standards of kashrus? Whose supervision is your institution under? Who is the moshgiach?

17) Is the dorm supervised by a mature adult resident? How long has the adult supervisor been with your institution? What is the supervisor's experience? Is the supervisor registered with the state?

18) Where does your institution place academically?

19) What defines an alumnus? How long must my child attend to be considered an alumnus?

20) Is your secular learning department modern?
Does the secular program have a modern video department? What about the computer lab and the English library? How recent are textbooks? (Please provide names, dates of publication.)


21) Is tuition negotiable? How many students pay full tuition in your institution?

22) Is the food always that good? (Please provide me with copies of your lunch menus for Shabbos and during the week.)

23) How does the staff behave?

24) Does your instutition meet its finanical obligations on time?
